Open the secure form in its own tab to enter company details and employees or dependents. Save a draft and keep its private return link if you need to finish later.
Open secure quote formAfter reviewing your information, select Submit to PDX Benefits. A receipt confirms that your request was saved and submitted. Saving a draft alone does not submit it. Please do not enter SSNs, medical history, or bank details.
What happens after you submit your request?
Your submitted information is saved directly for our team. We review your parameters and small-group carrier rate filings across Oregon, Washington, California, Michigan, or Arizona, and prepare a side-by-side proposal. Submitting a quote request does not enroll your business or obligate you to change carriers.
